Rates
$100 per 50-minute session
$125 per 50-minute EMDR session
$150 per 90-minute EMDR session

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health benefits?
• What is my deductible and responsibility for out of pocket cost?
• How many sessions per calendar year does my plan cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

I accept some insurances. Aetna, Cigna, Humana, UMR, CCW EAP, LifeSynch, Beech Street.
I DO NOT accept BCBS-FL or United Health Care but your carrier may be willing to reimburse you for partial payment as I meet the requirements as a specialist. You will need to call and inquire with your specific plan.

Reduced Fee
Reduced fee services are available on a limited basis for extenuating circumstances. You may be asked to supply documentation to meet this criteria.

Payment
Cash and check  are accepted for payment. All payments private pay and co-payments are due at the time of each scheduled session. There are no deferred billing options available. There will be a $25.00 charge for any check which does not clear or is returned for insufficient funds. If insurance returns a claim unpaid or partially unpaid and deems the insured responsible for payment I have the right to defer any appointment until the charge is paid in full.

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session and/or your insurance carrier may be billed which ever is applicable.
